some smp changes merged into click main branch - require new patch

hi all,

i merged some of the smp changes into the main click branch. the major 
difference is the patch. if you use linux click, you would need to re-apply
the 2.2.18 patch in click/etc to get the new polling interface. this interface
moves skb management (e.g. recycling) from device driver to click, so skb
recycling can be used with different device drivers. the device elements 
now use this interface instead of the old one. 

there are also some bug fixes and other small changes. but the patch and
the new device elements are the major changes.
